This project has resulted in reduced admissions, admission avoidance, reduced referrals, improved Hba1cs &amp; lipids, referrals to lifestyle interventions and 100% improved patient confidence.</p>

<p><strong>In a nutshell:</strong> The service is provided for the population of people with Type 2 diabetes in Newcastle within the North of Tyne Primary Care Trust. It was implemented in 2005 ot deliver the majority of diabetes care, for people with Type 2 diabetes in primary care. An integrated pathway of care included a Community Assessment Clinic (CAC) for GPs to access advice on management and referral for Insulin therapy, was also offered via a structured education programme.</p>

<p><strong>In a nutshell:</strong> NHS Somerset have developed a model of care involving patients, clinicians, commissioners and providers. The vision is for care to be more integrated and accessible, focussing on prevention, earlier diagnosis and support for self management. The model aims to deliver an equitable service across Somerset including: community based services, seamless care, educational programmes, healthy eating and physical activity programmes, management plans, access to specialist care and equality of access and choice. The majority of care for adult diabetes patients will take place in community settings, with specialist care (level 3) being provided in acute care hospitals.</p>

The QIP was implemented to promote compliance with international guidelines and included interventions such as the introduction of an Interdisciplinary Diabetes Care Team.</p>

The work on Diabetes care in the community was nominated for the TCS Innovation &amp; Leadership Award and won £85,000 funding to develop the service. the project will include training staff about self management; provide information for people with diabetes; distric nurses and GPs collaborating to identify people at high risk and detect new cases early.</p>

They conclude that continuity experiences, meanings and expectations, as well as barriers and facilitators, are influenced by the model of care rather than type of condition.<br />

<p><strong>In a nutshell</strong>:   This article reviews the use of oral hypoglycaemic agents (OHAs) from the standard Sulphonylureas (SUs) and biguanides (metformin) to the more recent thiazolidinediones, glinides, a-glucosidase inhibitors, glucagonlike peptide-1 agonists, amylin analogues and dipeptidyl peptidase-IV inhibitors. Whilst the safety of SUs and metformin have been established through long term use, clinical trials relating to the newer OHAs are needed to measure their risk benefit ratio against the standard regimens. It recommend that until established trials are available, patients should be treated on an individual basis weighing benefits and costs of individual medications.</p>

<p><strong>In a nutshell</strong>:   This RCT examines a self-management intervention to support patients recently diagnosed with type-2 diabetes to improve their self-care behaviours. The intervention is based on proactive coping to predict the maintenance of changes in behaviour, examining the role of anticipation and planning in maintaining self care. The intervention was successful regarding improved diet and exercise resulting in weight loss over 12 months. Proactive coping was seen to be a good predictor of long term self management.</p>

<p><strong>In a nutshell</strong>: The PREDICTIVE[TM] study, a large, multinational observational trial, was to evaluate the efficacy and safety of insulin detemir (IDet) in routine clinical practice. Twelve-week follow-up data from patients with type 1 (T1D) or type 2 (T2D) diabetes in the European cohort switched from once or twice daily insulin glargine (IGlarg) to qd IDet in a basal-bolus regimen. Switching from IGlarg to qd IDet was associated with improvements in glycaemic parameters and no increase in hypoglycaemic episodes or weight gain. Patients with T1D and T2D may be switched from IGlarg to qd IDet as part of a basal-bolus regimen.</p>

<p><strong>In a nutshell: </strong>This article outlines a trial about a decision aid to help patients with diabetes decide about statins (Statin Choice) and to determine whether it effects their trust in the clinician. This trial was blinded and included 98 patients with type 2 diabetes. This trial suggests that decision aids do not have a large impact on trust in the physician and may increase trust through improvements in the decision making process.</p>
