In a nutshell: This programme was developed to achieve the goals of World Class Commissioning including improved screening, increased awareness and management of Diabetes Type 2. The integrated team involved primary and specialist health care professionals working jointly with service users, public health, service managers and commissioners to achieve improved networking and partnerships. It includes practice visits, a support programme, structured education for people with diabetes, staff education and a social marketing campaign to increase awareness.
